Living Will and Health Care Power of Attorney (WV)
These documents allow you to appoint a representative in fact to make health care decisions for you when you become unable, and express your wishes for life-prolonging care in an end-of-life situation. Specifically, these documents are:
- Living Will, which lets you state your personal instructions for health care. Specifically, it lets you express your wish to withhold or withdraw life-sustaining treatment in an end-of-life situation when you are unable to make decisions about medical treatment.
- Medical Power of Attorney, which lets you designate a person as attorney in fact to make health care decisions for you when you become incapable.
