Living Will and Health Care Power of Attorney (WI)

This questionnaire helps you create a living will, also called a Declaration to Physician, and/or a Power of Attorney for Health Care, for a resident of Wisconsin.

1. Declaration to Physician (Living Will) - This lets you authorize a physician to make a decision on withholding or withdrawing life-sustaining procedures when you are no longer capable of doing it yourself.
2. Power of Attorney for Health Care - This lets you name someone to make a decision about life support, if you can no longer speak for yourself.
